The bonus objective to obtain the fourth super combo is located at the mission where you save the security guard in the church bell tower. As you climb the flights of stairs, you will encounter a group of 5 swat/police team which you Must kill in 30 seconds. The easiest way to accomplish this is to use Neo's Stop bullets/Charge objects ability using L trigger and the X button and hurling back a grenade at the team.
